Why Medigap premiums rise, and what a smart response looks like

Every Medigap policyholder gets increase letters, and almost nobody reads them properly. A premium increase is a filed, reviewed event with identifiable causes, and knowing them is what separates a rational response from panic or apathy.

Three ways a carrier prices you

Community-rated pricing charges everyone in the state the same rate regardless of age. Issue-age pricing fixes your rate band at the age you enrolled, so increases are smaller but the entry premium is higher. Attained-age pricing starts lowest and rises as you age, which is why the first few years feel comfortable and the later ones do not.

Attained-age is the most common method in most states, and it is also the one people misjudge. Those increases are not the carrier being difficult, they are the structure of the policy. When comparing two quotes, the question is not which is cheaper today but which pricing method you are buying.

What actually causes a filing

Four things drive the number. Medical trend, meaning the cost of care itself, rises every year for everyone. Interest rates matter because premiums are held in reserve before claims are paid, and a decade of low rates makes those reserves earn less. The size and health of your specific block matters, because a closed block with a shrinking population spreads fixed costs across fewer people.

Then there is the carrier decision to stay or leave. When an insurer stops writing new business in a state, its existing policyholders keep their coverage but the block can no longer bring in healthier new members. That is the mechanism behind the horror stories about a company raising rates three years in a row.

What to do when the letter arrives

Compare the increase against the market rather than against last year. A five percent rise on a policy that was already the cheapest option is still the cheapest option. A twelve percent rise on a policy that was mid-pack two years ago is a signal to shop, and shopping has a cost, because without a guaranteed issue right you must pass underwriting.

That asymmetry is the real reason we push people to buy the right policy the first time. Moving later is not impossible, but it is conditional on your health, and the people most motivated to move, those facing steep increases, are often the ones whose health history has changed since. There is no clever trick here, only buying well and reviewing annually.

Questions people ask about this

Can a carrier raise my rate because I got sick?

No. Increases must apply to an entire class of policyholders, not to an individual, and they require state approval. Your personal claim history cannot trigger a targeted increase.

Is the cheapest carrier the riskiest one?

Not necessarily. A low rate can reflect efficient underwriting or a carrier buying market share, and both can change. What we look at is the block size, how long the carrier has stayed in the state, and the last five years of approved increases on that exact plan.

Should I switch every time my premium goes up?

No. Each move means a new application and a new block, so switching should be reserved for real divergence from the market. We rerun the comparison annually and recommend a move only when the gap justifies the underwriting risk.
