Medigap

Medical underwriting for Medigap, without the scare stories

Outside your protected enrollment windows, a Medigap insurer is allowed to ask about your health before it sells you a policy. The process is more ordinary than the internet makes it sound, and it is far more predictable once you know what is actually being reviewed.

What underwriting is not

It is not a medical exam, and it is not a judgment about whether you deserve coverage. Nobody is coming to your house and no blood is drawn. It is a short set of questions on the application, checked against prescription databases and claims history, and its only purpose is to decide whether the insurer will accept the risk and at what price.

It also has nothing to do with your Medicare eligibility. Medicare itself cannot be taken away because you answered a health question badly. The worst outcome is that a particular carrier declines to sell you a supplement at that moment.

What they actually ask

Most applications ask about a defined list of conditions within a defined look-back period: recent cancers, stroke, heart attack, chronic obstructive pulmonary disease, diabetes treated with insulin, kidney disease, organ transplant, and sometimes memory impairment or the use of a wheelchair or oxygen at home. Height and weight, tobacco use, and current prescriptions round out the picture.

Blood pressure medication is normal and rarely disqualifying on its own. Height and weight matter only at the extremes. The questions that sink applications are usually about recent cardiac events, insulin-dependent diabetes in older age brackets, and active treatment for cancer. A good agent knows which carriers are softer on which conditions, which is the practical value of working with one.

How to prepare without gaming it

First, never cancel existing coverage before the new policy is approved and issued. Second, gather your prescription list and dates of major events, because a vague answer is more likely to be declined than a truthful specific one. Third, consider timing: if a condition is resolving or a treatment course is finishing, a later application may be treated very differently from an earlier one.

If you are declined, that is information rather than a dead end. Some carriers are more permissive, some states offer protections that eliminate the questions, and a Medicare Advantage plan may be the better fit in the meantime. What we avoid is stacking applications across multiple carriers at once, which leaves a trail nobody benefits from.

Questions people ask about this

Can I be declined for a Medigap policy?

Yes, outside a guaranteed issue situation. During your six-month open enrollment period, or under a federal trial or loss-of-coverage right, an insurer cannot ask about your health or turn you down for it.

Does taking a medication for a condition automatically disqualify me?

No. Carriers look at combinations and severity. A single blood pressure medication is routine; a set of medications indicating recent intensive treatment draws more scrutiny.

If I am declined, can I try another carrier?

You can, though it is worth understanding why the first declined before spending effort. Different carriers treat different conditions differently, so a more targeted second application is usually smarter than a broad one.
