Medicare Savings Programs and Extra Help: who actually qualifies

Medicare sets its premiums nationally, but what an individual household pays for them is negotiable through four separate assistance programs. Most people who qualify never file, largely because the income limits sound lower than they are and the paperwork never gets explained out loud.

The limits are higher than the rumor suggests

Medicare Savings Programs are administered by state Medicaid agencies under federal minimum standards, and most states set their limits above the federal floor. Countable income is not the same as gross income: a portion of what you receive is disregarded, and the thresholds rise with the size of your household even when only one person is applying.

Asset limits are the other half of the test, and they are more forgiving than they look because a primary home, one vehicle, and basic household goods are generally excluded. A savings account large enough to feel disqualifying on paper is often well under the line once exclusions are applied. This is the single most common reason a person assumes they are ineligible and never asks.

Four programs, each covering something different

Qualified Medicare Beneficiary is the widest one. It pays the Part B premium, and it also covers Medicare cost sharing such as the Part A deductible and the Part B coinsurance, while preventing providers from billing you the difference on covered services.

Specified Low-Income Medicare Beneficiary and Qualifying Individual are narrower: they pay the Part B premium, and the second operates through a capped annual block of federal money that runs out in some states before the year ends. Extra Help, also called the Low-Income Subsidy, is separate and applies only to prescriptions, lowering or removing the Part D premium and shrinking copays. Enrollment in a savings program qualifies a person for Extra Help automatically, which means one application can be worth more than it first appears.

Where to file, and the trap at the end

The savings programs are filed with your state Medicaid office, while Extra Help can be filed through Social Security. Both require proof of income, and both must be renewed, usually every twelve months. Missing a redetermination letter is the usual way people lose a benefit they still qualify for.

There is one counterintuitive consequence worth knowing before you apply. If Qualified Medicare Beneficiary status takes over your cost sharing, a Medigap premium is largely paying for benefits someone else is already covering. We review that before recommending a supplement to anyone receiving this help.

Questions people ask about this

Does taking this help mean I am on Medicaid?

It means you are enrolled in a Medicare Savings Program, which is run through the Medicaid agency and uses its income tests. For most people the practical effect is help with Medicare premiums and cost sharing rather than full Medicaid coverage, but the enrollment is a Medicaid program and some states treat it as such in their records.

Can I lose my house because of this?

Estate recovery is a rule attached to certain Medicaid long-term care benefits, and states have limited authority to recover against the estates of people who only received Medicare Savings Program help. Rules differ by state, so ask the state office directly rather than assuming either answer.

I already have a Medigap policy and just qualified. Now what?

Keep paying it while you confirm what the state covers. Many people drop the supplement and later lose their assistance, at which point they must pass medical underwriting to buy it back. We would rather see the premium reviewed carefully than the policy cancelled quickly.
