When you have both Medicare and Medicaid

Two programs covering one person sounds like duplication. It is not: they pay for different things in a fixed order, and understanding that order is what stops a hospital bill from arriving at the wrong address.

Medicare pays first, always

Medicare is the primary payer for a person who has both. It processes the claim, pays what it covers, and only then does Medicaid consider the remainder. That is why a provider needs both cards, and why a claim that was never sent to Medicare will not be picked up by Medicaid later without a fight.

What Medicaid picks up depends on the level of eligibility. People with full Medicaid have their Medicare premiums and cost sharing covered almost entirely. People in a Medicare Savings Program have narrower help, usually the Part B premium and some cost sharing. The label on the letter matters more than the fact of being enrolled.

What a D-SNP really is

A Dual Eligible Special Needs Plan is a Medicare Advantage plan restricted to people with both coverages. It bundles medical benefits with Medicaid services and often adds dental, transportation, and over-the-counter allowances, which is genuinely valuable for a population that struggles with those costs.

The catch is the network. Accepting a D-SNP means receiving care through that plan rather than through original Medicare, and the plan can change its provider list and its extra benefits every January. Anyone considering one should confirm their specialists are in network for the coming year, not just the current one.

The renewal is the whole system

Medicaid eligibility is redetermined regularly, and a change in income, a move, or an unopened envelope can end it. If Medicaid ends, the D-SNP typically ends with it, and the person is left needing standard Medicare coverage without the assistance that made it affordable.

The good news is that losing Medicaid creates a guaranteed issue right to buy a Medigap policy without health questions, provided you apply within the deadline and can document the loss of coverage. It is a narrow but real safety net, and it is much easier to use when someone knows it exists before the letters arrive.

Questions people ask about this

Should I buy a Medigap policy too?

Rarely while full Medicaid is covering your cost sharing, since the premium would be paying for a gap someone else already fills. The exception is a household that expects to lose Medicaid eligibility and wants protection in place, which is a conversation to have before the change happens.

Can I keep original Medicare and still get Medicaid help?

Yes. Plenty of people with both coverages stay in original Medicare and use Medicaid to absorb the copays. Nothing forces you into a special needs plan.

What happens to my prescriptions if I lose Medicaid?

You would need a stand-alone Part D plan, and losing Medicaid gives you a special enrollment period to join one without a late penalty. Do not wait for the pharmacy counter to raise it, because the transition is where people go without medication for a week.
